Transition from Child to Adult Services

We are dedicated to supporting young people transitioning from child to adult mental health services, a time that can be complex and challenging. We offer a range of services to ensure that this transition is as smooth and supported as possible.

Our advocacy services help young people and their families navigate the differences between child and adult mental health care. We assist in understanding new systems, accessing appropriate care, and ensuring that each individual’s needs are met.

We provide emotional support through one-on-one counselling and peer mentoring, offering a safe and confidential space to share concerns and experiences. This helps reduce the anxiety that often accompanies this change.

Our charity also offers guidance and resources tailored to the transition process, including workshops on managing mental health, building resilience, and understanding the differences in care models.

To further promote well-being, we organise community activities and social support groups to reduce isolation, foster peer connections, and create a sense of belonging during this crucial period. Our services are designed to empower young people with the skills and confidence to manage their mental health as they move forward into adulthood.
